Care Coordination
Parents who have children with disabilities and/or special health care needs often have multiple case managers who assist with coordinating various aspects of their child’s care. Yet, parents often have questions about how this process works.
- There seem to be many different models of care coordination. How do I know which model(s) will best meet the needs of my child and family? How do I work with multiple care coordinators across different models?
- I am unsure of how a care coordinator can assist my family. What are the roles and responsibilities of a care coordinator? Are care coordinators certified? Are they effective?
- I recently read about care coordination, but none of the care that my child gets seems to be coordinated. As a parent, how do I advocate to obtain care coordination?
